For home health and hospice agencies, staying connected with patients between visits is a care imperative.

nanaCONNECT - ENGAGE was built on that belief, enabling agencies to reach patients daily through simple SMS check-ins and a straightforward 1-5 rating system. That consistent touchpoint helps teams detect changes early, trigger timely follow-up, and keep patients at the center of their care.

Now, ENGAGE takes that connection a step further. With the rollout of its new webform feature, agencies can go beyond "how are you feeling today?" and start asking the questions that matter most to their specific patient population.

What Are Webforms?

Webforms allow agencies to embed structured questionnaires directly into their patient messaging workflows. Through ENGAGE, a unique, secure link is automatically included in the SMS sent to a patient. That link opens a branded, mobile-friendly form with custom questions and multiple-choice answer options—all of which can be configured by the agency ahead of time.

The process is designed to be frictionless for patients. There are no logins, no portals to navigate, and no apps to download. Patients simply tap a link, answer a few questions, and submit. Each link is unique to the individual, encrypted for security, and valid for 14 days, so there's a reasonable window for completion without the data becoming stale.

On the agency side, webform results flow into a centralized dashboard where teams can view responses by form name, monitor submission counts, and export data for reporting. Dynamic columns in the export reflect each form's specific questions, keeping the data clean and analysis-ready without manual reformatting.

Built for Real-World Use

A few thoughtful design decisions make ENGAGE's new webform feature especially practical for care-at-home agencies.

First, webforms lock once they've been used to send messages. This ensures that the questions patients receive stay consistent throughout a campaign, meaning no accidental edits that could compromise data integrity or create confusion for patients mid-cycle.

Second, automated SMS reminders go out exactly seven days after the original message when a patient hasn't yet submitted a response. The reminder uses the same webform link, so nothing needs to be regenerated or re-sent manually. This reduces the staff burden of manual follow-up while giving patients a nudge at a natural interval.

Third, agencies can test before they deploy. A demo mode lets teams send a test SMS with a preview of the webform—questions, branding, and all—without capturing any real submissions. That kind of visibility into the patient experience before go-live is the kind of detail that makes a difference in adoption and confidence.

Why It Matters

The 1-5 daily rating has always given agencies a real-time pulse on patient well-being. Webforms add dimension to that signal. Instead of just knowing a patient rated themselves a 2, agencies can now understand what's behind that number—whether it's pain management, equipment concerns, a question about their care plan, or something else entirely.

That richer data enables more targeted interventions, informed conversations between clinicians and patients, and ultimately, better outcomes. For agencies managing large patient populations across multiple branches, the ability to filter, export, and analyze response data at scale is a meaningful operational advantage.

ENGAGE has always been about keeping patients connected to their care teams in a way that feels simple and human. Webforms build on that foundation, turning daily check-ins into a genuine two-way conversation.
